Referrals
If you are a Physician, Specialist or a Healthcare provider and would like to refer your patient to Sudha Psychotherapy for therapy and they have provided consent, please follow the instructions below:
STEP 1 Please download the referral form and complete the information pertaining to the patient you are referring to us.
STEP 2 Click ‘Choose File’ tab to upload the completed referral form
STEP 3 Click on ‘Upload Form’ button
